Yoga helps Mass. veterans cope with PTSD

Michael Dziokonski returned from the Vietnam War and quickly got involved in life. He went to graduate school at Boston College, became a social worker, got married, raised four children, got elected to the Clinton Board of Selectmen and over the past years, has watched three wars unfold on his television screen.

Dziokonski, who was a Navy corpsman assigned to a Marine Corps rifle company in Vietnam and a recipient of two Purple Hearts and a Bronze Star with a Combat “V,” considers himself a strong man, a self-described curmudgeon.

So when he found himself at the top of a mountain crying over a dead butterfly, he knew he needed help.

And he found relief and release in a yoga program designed specifically for war veterans called Yoga Warriors International.

Yoga Warriors International was developed by Lucy S. Cimini, the founding director of Central Mass Yoga and Wellness in West Boylston, designed to alleviate the symptoms of combat stress or post-traumatic stress disorder.

“Yoga does wonderful things for PTSD,” Dziokonski said. “It introduces other thoughts into the mind. It’s another way of thinking. Yoga introduced me to mindfulness. It stresses present-moment awareness. With PTSD, your emotions are ruled by what happened 20 or 30 years ago.”

Dziokonski originally took yoga to gain more flexibility. As he practiced yoga, the wars in Afghanistan and Iraq were unfolding a world away, but were hitting close to home for Dziokonski.

“I was bothered a great deal. The war was in our living room,” Dziokonski said, recalling the firefights that were being broadcast on the nightly news. He said he found himself tense, angry, and prone to overreaction.

Cimini developed the Yoga Warriors program in 2003. It grew out of her work with veterans who were attending her classes. In fact, Dziokonski had asked her to teach a class at the Worcester Veterans Center, which fellow veterans later decided to continue.

The program is designed to help alleviate post-traumatic stress disorder in veterans. It focuses on these four principles: incorporating positive affirmations with specific postures, because thoughts influence health; breathing that focuses on calming and diaphragmatic breaths, with the understanding that the mind follows the breath; regarding yoga as a relaxing experience, not a competition; and creating a sense of safety.

Cimini travels the country training yoga instructors in Yoga Warriors International. In addition, she is the co-author of a study that will appear in the American Journal of Occupational Therapy, “The Effects of Sensory Enhanced Hatha Yoga on Symptoms of Combat Stress in Deployed Military Personnel.” The study was done by Maj. Jon Greuel, an Air Force instructor pilot, in Kirkuk, Iraq, from November 2008 to November 2009 in collaboration with Cimini.

She is also working with Worcester State University to study the effect of the Yoga Warrior program on military students at the university.

Her program has received inquiries from as far away as Germany, Sweden and England.

Several yoga instructors are trained in the Yoga Warriors program. Zest Yoga and Fitness in Auburn is one of the local studios offering the Yoga Warriors class with a certified instructor.

Zest Yoga and Fitness in Auburn began offering a Yoga Warriors class in September, and is open to all veterans and active-duty military personnel.

The poses incorporate a lot of linear movements, such as “extended cat” and “downward dog.” Positive thoughts are matched with specific poses for instances “I grow from strong roots” is repeated in “tree pose.”

There is no pressure to discuss feelings or reveal anything about oneself to others.

Dziokonski, who regularly attends a Thursday morning Yoga Warriors session, said yoga has returned to him a sense of calm, and has improved his sleep. Many Vietnam veterans are enrolled in Yoga Warriors, and also some World War II veterans, and now Iraq War veterans.

Dziokonski and Cimini said the emotional impact the war has on returning veterans may not become evident until months or years later.

Children, on the other hand, are by nature open and imaginative.

The facilitator of the child’s experience determines the success his meditation. Anyone can become a dynamic child meditation facilitator, whether they are a parent, teacher or therapist. The facilitator should be familiar with the most important benefits a child attains through meditation, practice and awareness. When a child meditates, he practices something in his head so later it is easier for him to carry out the particular action or feeling. Meditation also brings into consciousness thoughts the child is unaware of during normal consciousness.

Practice: something we do so we feel better later

When we practice a process in our minds, we are able to successfully repeat this process in the real world. Simply stated, we do something in our heads so we feel better later. This is similar to working out our body at the gym. We strengthen our biceps so we are able to lift a heavy box later that week. Likewise, during med itation, seven-year-old Ashley practices how to feel and the let go of her anger during a grounding meditation. Because she has practiced this during meditation it is easier for her to process her anger later.

The most ideal practice meditations bring calmness and focus to a child. These meditations are quite grounding, that is they bring a child into balance with the natural rhythms of the earth, which results in physical stability and emotional ease. A child can practice these meditations absolutely anywhere, even in her classroom when she begins to feel distracted or frustrated.

An effective awareness meditation is the Happy Tree meditation, during which the child sees a tree with happy and sad fruit. This is a metaphorical journey for the child. In many ways the sad fruit and the tree represent her sadness and the happy fruit represent her happiness. During the meditation, the child nurtures her sad fruit in a special way. Since this is a metaphorical journey, the parent and child can become aware that ways in which she nurtures her sad fruit can also help her understand how she can nurture herself.

Yoga a great treatment for low back pain

When I started having low back problems, I thought of swimming and other low impact exercise. However, yoga was not something I considered.

This was partially because of my ignorance of yoga and my admitted prejudice about a discipline that includes postures called Downward Facing Dog, Half Moon, Fetus, Cow Face and others.

However, a study published recently in the Annals of Internal Medicine journal has suggested that yoga is more effective at decreasing lower back pain than conventional medical treatment.

The study assigned 156 patients to yoga classes and a control group of 157 patients to standard care. The control group received a range of interventions, including mild pain relief medication, physical therapy and advice to remain active and avoid heavy lifting.

On average, members of the yoga group were able to undertake 30 percent more activities compared with those in the usual care group after three months.

On the other hand, yoga did not yield greater reductions in pain or improvements in overall health compared with usual care.

In terms of yoga studies for low back pain, this study is the largest so far, with over 300 participants, and it has the longest follow-up of any yoga trial.

The participants were followed for nine months after they had finished the yoga classes and found that the benefit of yoga was sustained for this length of time, probably because many of the participants continued to practice yoga after they had completed their formal course.

The study participants were offered Iyengar yoga classes (12 classes total, once weekly). The yoga classes were given by 12 yoga teachers who had extra training in back care. Each class lasted 75 minutes.

Part of the idea behind this study is that yoga aims to treat the whole person rather than just the physical aspects of back pain. According to one of the investigators, the yoga program included poses for pain-relief and mental calming; mobilizing, stretching, strengthening and relaxat ion; imp roving awareness of posture; education about how a healthy back functions; and positive mental focus.

If you think the yoga effect sounds too good to be true, you might be correct in some cases. Eight percent of yoga participants reported adverse events that were mostly nonserious and generally related to increased pain. Adverse events were reported in only one percent of the usual care group.

Other interventions for low back pain that have been previously evaluated in randomized controlled trials include exercise and manipulation, the Alexander technique, and cognitive behavioral therapy.

Compared with these other techniques, yoga may improve back function more than exercise and manipulation, cognitive behavioral treatment, and 6 sessions of 1-on-1 Alexander technique but not as much as 24 sessions of Alexander technique.

From my point of view, I believe that this study reinforces the idea that back pain is best treated with activity and gentle stretching, rather than rest, except for a short time after an acute injury.

If you are a back pain sufferer, do include yoga as you consider physical therapy, low impact exercise and stretching, and other treatments.

Most of all, try to keep moving as much as you can. As I explained to my mother (usually during church) when I was a child, God does not want us to just sit still.
